Samantha Cole, of Rodbourne Road, Swindon, was well known at the town's Great Western Hospital, the place where she died earlier this month.
Sammy, who had been born with the terminal illness cystic fibrosis, died in the arms of her mother Lisa Cole. She had just turned 18.
As a child, she was often a patient in the hospital but in recent years her mother had taken over her care and learned to give Sammy her daily injections.
She was taken into hospital with penumonia in February and, on March 12, her consultant warned the family that Sammy's time was running out.
Everything was arranged for the teenager to go home so that she could be with her family.
But at 4am the following day the family was asked to come to the hospital and Sammy died two hours later.
